Long Term Care


Eligibility

All full-time employees regularly scheduled to work at least 30 hours per week are eligible for Long Term Care coverage on the first of the month after completing two full calendar months of employment.


What is Covered

Long Term Care is the assistance received when someone needs help with two or more Activities of Daily Living – such as dressing, bathing, going to the bathroom, eating and moving about – or when someone suffers a severe cognitive impairment. This could be provided in the home, in an assisted living or residential care facility, or in a skilled nursing facility such as a nursing home.

J M Smith is proud to provide you with a base level of benefits to help you meet the needs that Long Term Care can create. J M Smith is providing a benefit amount of $1,000 per month payable for up to 3 years. In addition, J M Smith has designed a plan that will allow you to purchase additional coverage to enhance the benefits provided to you.


Cost of Benefit

J M Smith is paying 100% of the basic plan coverage based on your age as of January 1, 2008 (original plan effective date), for employees employed with the company when the plan became effective.
Insurance age for coverage for employees employed on or after the plan effective date, is your age on the date you are effective for the benefit.

Continuing Coverage After I Stop Working

You may continue coverage at your own expense. Certain conditions apply. You will be provided with the proper forms, information, and costs upon leaving your employment with J M Smith Corporation. Contact your Human Resources Department for more information.
